 CAHC is open Monday through Friday, 8:30 am to 5:30 pm.  Hours are extended on Wednesdays from 7:00 am to 5:30 pm to help accommodate community needs.  Patients can schedule regular appointments during the extended hours and special services like labs and x-rays.  Call (208) 879-4351 to schedule an appointment. 

We offer same day appointments for your convenience.

 Make sure to bring the following with you for check-in: 

  • Photo ID
  • Insurance Card
  • Emergency Contact Information
  • All medication and dosage, including over the county and herbal
  • A list of known allergies
  • Any questions or concerns you have about your health
  • Any forms, logs, checklist needed. You can find those on the forms page

 To provide the best care possible we will review and update all insurance information, listed allergies and medications at your arrival via our IPad check in system. 


A nurse will go through your health history as needed with any changes to surgical, family or social history.

​​We pride ourselves on coordinating care. To effectively care for your overall health we ask that you provide information you may have from other medical providers. We encourage you to choose a personal provider, this ensures your care is consistent and coordinated. Education and self-management resources are available.

The Challis Area Health Center, Inc. is a Federally Qualified Health Center (FQHC) and receives federal HHS funding through its designation as a Section 330 community health center.  Related to CAHC’s status as an FQHC, the CAHC has federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims for itself and its covered individuals (i.e., covered by the Federal Tort Claims Act/FTCA).  For more information, please see visit HRSA.gov.
